Special Project Application Form

Deadline: Monday, October 7, 2019 at 5 P.M.


  1. Read ALL of the GUIDELINES.
  2. Completely fill out the form below and budget Excel sheet (Click Here to Download)
  3. Attach a maximum two-page detailed description of your project along with your budget using the upload link below. Both the budget and the description must be in one file.
  4. Review the form to ensure that it is complete.
  5. Submit the form.

COMPLETE all information on this form. Incomplete applications will not be considered.

The following supporting documentation must be submitted with the application:- SEE CHECKLIST BELOW

  1. A maximum 2-page detailed description of your project
  2. A balanced budget, listing all revenues and expenses (anticipated and confirmed)

CCSL Special Projects Application Checklist

Please note that incomplete applications will not be submitted to the CCSL Special Projects committee for review.

  • Completed application form
  • Maximum 2 page detailed description of your project as uploaded PDF attachment
  • A detailed, balanced budget listing all revenues and expenses (anticipated and confirmed) included in uploaded PDF attachment
  • The budget shows evidence that other sources of funding have been confirmed or requested
  • The budget is balanced, with revenues equaling expenses
  • Amount requested from CCSL does not exceed 50% of the project’s total expenses
